(b)Photocopy of Caste (S.C/S.T, OBC/ Minority) /Handicapped certificate (if any) duly attested (Certificate from either D.M/S.D.O/ M.P/ M.L.A/Municipal Councillor/Member, ZilaParishad only valid for S.C/S.T. candidates).Sports certificates from competent authority to be submitted. (For reserved category).

(Original & Photocopy without attestation - two sets)

(a) Migration Certificate (for migrating students only). (b) H.S. or equivalent Mark sheet & admit card (c) Madhyamik Admit card. (d) Two copies of Stamp-size photograph (coloured). (e)Photocopy of Caste (S.C/S.T, OBC/ Minority) /Handicapped certificate (if any) duly attested (Certificate from either D.M./S.D.O./ M.P./ M.L.A./ Municipal Councillor/ Member, ZilaParishad only valid for S.C./S.T. candidates). Sports certificates from competent authority to be submitted. (f) Income Certificate for BPL candidate only from competent authority. (g) Certificate for Break of Study (if any). (h) School leaving certificate .
